GSF was developed over five years with a multidisciplinary reference group and pilot projects. It aims to improve palliative care provided by the whole primary care team and is currently being used by 1000 practices in all cancer networks. It enables those approaching the end of life to be identified, their care needs assessed, and a plan of care with all relevant agencies put in place. The framework focuses on optimising continuity of care, teamwork, advanced planning (including out of hours), symptom control and patient carer and staff support. Although developed for use in primary care it can be used in care homes and for all disease groups.

Each project is run by a local facilitator supported by the National Team and their network/SHA, a local GSF project team will be developed to adapt and develop the work locally.
Although developed for use in primary care with cancer patients GSF can be used in other settings and for all disease groups. A pilot project in care homes is underway.

The Gold Standards Framework (GSF) is a framework to enable a gold standard of care for all people nearing the end of their lives. GSF is a systematic evidence based approach to optimising the care for patients nearing the end of life in the community. It is concerned with helping people to live well until the end of life and includes care in the final year of life for people with any end stage illness in the community.
