Preferred priorities for Care

Formerly Preferred Place of Care, an example of Advance Care Planning (ACP).

This patient held document was designed to facilitate patient choice in relation to end of life issues. Through good communication and by documenting patient and carers choices, they become empowered through the sharing of this information with all professionals involved in their care. The PPC provides the opportunity to discuss difficult issues that may not otherwise be addressed to the detriment of patient care. The explicit recording of patients/carers wishes can form the basis of care planning in multi-disciplinary teams and other services, minimising inappropriate admissions and interventions.

The PPC also records services available, services being accessed and reasons for changes in the care trajectory. PPC is a process, which facilitates service review, further empowering professionals to negotiate service requirements on the behalf of patients, becoming an integral part of service commissioning and design.

The PPC Network is currently being piloted in the care home sector both for older people and for people with acquired brain injury. Work is continuing to address the needs of patients in the acute hospital sector.

Early evaluations suggest that the PPC is having a significant impact on patients receiving care in their preferred place of care at the end of life. This has been achieved without significant changes in service delivery being required.

Preferred Priorities for Care: West Essex Evaluation (PDF)

NHS West Essex has pledged to ensure, wherever possible, that the care provided to people at the end of their lives will meet their individual needs, and that people will have real choice about where and how they receive that care. This document is an evaluation report on how successful PPC has been and outlines what improvements can be made.
